Care Credit Patient Payment Plans
CareCredit is a medical credit card program that gives patients another payment option. You can use the credit card to pay copayments, deductibles, non-covered services or to pay off balances. We offer interest free payment options along with reasonable extended payment plans.
There are 3 ways you can sign up for CareCredit:
1. You can contact CareCredit by phone by calling 1-800-677-0718 and answer simple application questions. You will be notified immediately about approval.
2. You can go online at CareCredit.com and apply online. You will be notified immediately about approval.
3. You can complete an application while at the clinic. Azhar Therapy & Fitness will not have approval for 2-3 days but we can process the application. Please bring two pieces of identification to complete the application.
